Sixty-five of the 74 catalog codes are literally present in its CPT-4 and HCPCS columns across 240 rows, but the shipped extractor returns ZERO rows on BOTH engines: BareChargemasterHandler does not detect the file because it requires a literal cpt header and a gross-charges column, and CMSHPTHandler claims the header and finds no code|N pairs. This is a FORMAT limitation on a genuine posting, not a missing posting and not a wall.
