Template 1.1.0 nests code lines under `billing_code_information` while lib/sunlight/mrf/json_extractor.rb reads only `code_information`, so every row is skipped and 0/74 catalog codes extract. The rest of the v1.1.0 schema (standard_charges / gross_charge / discounted_cash / setting / billing_class) matches what the extractor already expects.
